01整体情况
1、基本检查情况
2022年8月东京备忘录地区共开展PSC检查2470艘次,发现缺陷4758项,滞留船舶82艘次,滞留率为3.32%。
2、船舶滞留情况
8月份13个国家/地区实施了滞留,中国滞留船舶艘次最多,共滞留了28艘次,澳大利亚滞留了21艘次,俄罗斯和日本各滞留7艘次。
滞留率最高的国家/地区依次是:巴布亚新几内亚,8.33%; 澳大利亚,7.69%; 中国,7.67%;巴拿马,5.88%; 俄罗斯,4.67%。
3、滞留缺陷类别分布
8月份共开出滞留缺陷136项,占据首位的是消防安全缺陷,排名二、三位的依次是防污染和ISM。

1、应急发电机储备燃油量不足
缺陷描述:
The quantity of fuel oil stored in fuel oil tank located in E/G room cannot keep emergency generator running 18 hours.
应急发电机燃油柜储存的燃油量不能满足应急发电机运行18小时的要求。

The emergency source of electrical power shall be capable, having regard to starting currents and the transitory nature of certain loads, of supplying simultaneously at least the following services for the periods specified hereinafter, if they depend upon an electrical source for their operation:
2.2 For a period of 18 hours, emergency lighting:
2.3 For a period of 18 hours, the navigation lights and other lights required by the International Regulations for Preventing Collisions at Sea in force.
2.4 For a period of 18 hours:
2.5 For a period of 18 hours one of the fire pumps required by Regulation II-2/4.3.1 and 4.3.3 if dependent upon the emergency generator for its source of power.

SOLAS公约1981年修正案要求船舶应急发电机的燃油必须至少能保证需要应急发电机提供能源的应急照明灯、航行灯、消防泵等设施运行18小时。PSC检查中发现,应急发电机燃油柜储存的燃料不满足于支撑相关设备运行18个小时,究其原因是多次运转测试后未及时补充燃料,且船员日常疏于检查导致未及时发现该问题。
2、系固设备未经认可
缺陷描述:
Numerous mobile securing devices supplied to the ship on 20 July 2022 not included in the approved cargo securing manual, also, the type of those securing devices are not the type listed in the CSM which approved by RO.
2022年7月20日供应上船的大量移动绑扎设备没有记录在经认可的《货物系固手册》内,且这些设备的型号不是经认可的《货物系固手册》里列明的型号。

S74-26/VI/R5
All cargoes, other than solid and liquid bulk cargoes, cargo units and cargo transport units shall be loaded, stowed and secured throughout the voyage in accordance with the Cargo Securing Manual approved by the Administration. The Cargo Securing Manual shall be drawn up to a standard at least equivalent to relevant guidelines* developed by the Organization.

This sub-chapter should describe the number of and the functional and design characteristics of the portable cargo securing devices carried on board the ship, and should be supplemented by suitable drawings or sketches if deemed necessary. It should contain the following information as applicable:
2.2.1 a list for the portable securing devices, which should be supplemented with appropriate documentation for each type of device, as far as practicable.

SOLAS2002年修正案要求《货物系固手册》的编制标准应至少等效于本组织制定的指南*。该指南要求:船舶在整个航程中必须按照《货物系固手册》的要求对货物进行绑扎系固,《货物系固手册》必须清楚描述船上使用的移动绑扎设备的数量、功能和特性,且手册必须包含每一个型号移动绑扎设备的清单。现场检查中经常发现船上系固绑扎设备型号与经认可的《货物系固手册》里列明的型号不一致、新供应上船的系固绑扎设备未在手册中及时登记等问题,请相关公司和船员对上述情况加以注意。
3、机电设备故障
缺陷描述:
The ship presented a danger that she unable to be maneuvered freely at BAOSHAN CHANNEL, area of high density and narrow water on 16th July 2022. It is happened that M/E can not be started ahead and astern at both of ECR and engine side. The main engine is not able to be started until PSC inspection. The root cause of this failure and precautionary measures need to be dug out.
2022年7月16日,该船在宝山航道高密度狭水道出现无法自由操纵的危险。主机在集控室和机旁无法正车和倒车,直到PSC检查时主机仍然不能起动。需要找出故障的根本原因和预防措施。

Regulation 26
General
1 The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and
protected as to reduce to a minimum any danger to persons on board.
2 The Administration shall give special consideration to the reliability of single essential propulsion components and may require a separate source of propulsion power sufficient to give the ship a navigable speed, especially in the case of unconventional arrangements.
3 Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative.
Regulation 28
Means of going astern
1 Sufficient power for going astern shall be provided to secure proper control of the ship in all normal circumstances.
分析:
本例是一起典型的船舶机电故障案例。根据SOLAS要求,船舶主、辅机应满足预定用途,航行中船舶应能保持足够的可操纵性。船舶在恶劣海况、高密度船舶流的狭水道发生机电故障,会对海上人命财产安全造成巨大威胁。为保障上海港水域安全,加强在航机电设备故障船舶安全管理,上海海事局发布了《关于加强机电设备故障船舶安全管理的通告》,请相关航运公司(船舶所有人、经营人或管理人)切实履行安全生产主体责任,督促船员履行安全管理责任,船长应对船舶机电设备进行自查,确认船舶处于适航状态。
04部分滞留缺陷
1. Several sections of fixed CO2 release pipe for cargo hold holed.
2.Emergency fire pump engine failed to start.
3. The clutch and control handle of windlass rusted seriously.
4. Plenty of fixed lashing plates on top of hatch covers wasted and corroded seriously.
5. Side rope of starboard side pilot ladder was broken on 29 July, 2022 during the embarkation of the pilot, which poses a danger for the embarkation/disembarkation of the pilot.
